In order to allow you to fill out and submit your SFS electronically, we will be using a secure online electronic signature service provided by a third party DocuSign, Inc. (the "DocuSign System"). Should you choose to use the DocuSign System, both you and EBS d.a.c and/or EBS MF (as may be relevant – the “Bank”) will sign this SFS by using an electronic signature (the “Signature”). The personal data contained in the SFS and the Signature, will be captured by DocuSign, Inc and shared with the Bank. DocuSign, Inc will store the SFS and the signature, for no longer than necessary. If you do not wish for your personal data and signature to be captured, processed and stored by DocuSign, Inc please do not proceed with filling in your details.
A security code will be sent to your mobile, so we can make sure your form is going to the correct person.
